All faculty must complete the following program deliverables in order to complete checkout:

Final Research Project Report

• Upload to your Google drive folder
• File name format: “VFP_Summer_YEAR_Research Project Report_Smith__John.pdf  Adobe Acrobat(.pdf) file format
• Log into the DOE WDTS website click on “Deliverables” tab and upload research project report to “Research Project.” (VFP only)

Final Oral Presentation

• Upload to your Google drive folder,
• File name format: “VFP_Summer_YEAR_Oral Presentation_Smith__John.pdf  Adobe Acrobat(.pdf) file format
• Once you receive approval from Nakeiah Harrell, log into the DOE WDTS website click on “Deliverables” tab and upload research oral presentation to “Oral Presentation.” (VFP only)

WDTS Post-Survey (VFP only)

• Complete survey on the DOE WDTS website. If you require assistance, please send an email to SC.WDTS@science.doe.gov.

Workforce Development & Education Surveys

All surveys are to be completed online prior to checkout.

• Workforce Development & Education Program Evaluation Survey ( administered via www.surveymonkey.com ) – An email invitation will be emailed to your LBL address the week of the program.
• Workforce Development & Education Mentor Evaluation Survey (administered via www.surveymonkey.com) – An email invitation will be emailed to your LBL address the last week of the internship

Berkeley Lab Career Profile

• Visit jobs.lbl.gov, scroll down the page and select grey button labeled “Go to Profile.” Please use a non-LBNL email address to create your profile, so that you can still access it after the end of internship.

Mentor Check-out Form

The Mentor Check-out Form must be completed by your Mentor or Associate Mentor.

Transfer Request

If you would like to continue your research with your LBNL mentor after the last day of the program, please have your mentor work with their HR division partner to determine the job classification and rate of pay prior to initiating the transfer to your division.

Missing online courses are to be completed before checkout.VFP Faculty are allowed to keep their badge for the duration of the three summer commitment. All participants will receive their final stipend when all program deliverables have been met. If program deliverables are not met by the end of checkout, the final stipend will be mailed to participant when all program deliverables have been met.
